Traditional Plan - Comprehensive Master Medical
The traditional plan covers all eligible hospital, medical and surgical expenses subject to an annual deductible and co-payment. Plans are available to one-subscriber groups (sole proprietors) and groups of two or more. Features of the plan include no medical underwriting; rates based on one person, two persons, family and family continuation; optional riders and more.

Community Blue PPO
The Community Blue PPO combines the benefits of traditional care with the wellness and prevention features of managed care. Use in-network physicians for low out-of-pocket expenses; or you may also elect to use out-of-network physicians for higher out-of-pocket expenses. Features of this plan include, but are not limited to, the following: flexibility as in- and out-of-network care is covered; basic benefits are covered with no deductible and no percentage co-pay.

Blue Care Network (HMO)
This plan emphasizes preventative care, and includes services for hospital care, physician services, diagnostic testing and prescription drugs with minimal co-payments. Some of the features of this plan are managed health care; choice of nominal co-pays per service; basic benefits covered with no deductible or co-pay; and preventative services.

High-Deductible Health Plan
The high-deductible health plans meet specific federal requirements related to deductibles and out-of-pocket expenses. Deductibles are determined each year by the federal government and may change based on increases in the Consumer Price Index. No medical underwriting; rates based on one person, two persons, family and family continuation; and eight geographical rating areas in Michigan are some of the features of this plan.
